How to submit a request
Use the contact channel published on the website and state “privacy request.” Do not send medical information in the first message. Only the minimum information needed to verify identity and identify the request will be requested.
Identity verification
We must avoid disclosing data to an unauthorized person. Limited verification information related to contact or appointment details may therefore be requested. We will not ask for a password or secret verification code.
Requests for minors
A parent or legal guardian should submit the request and provide only the evidence of authority required by law.
